Made changes to fix `KeyError` and implemented logging
of skipped tracks. For instance:
if any tracks are skipped, the log will show:
`Health status: Some tracks were not ripped (skipped)`.
the tracks that are skipped will show:
`Status: Track not ripped (skipped)`.

This fixes a regression from move to ruamel.yaml where all other fields in rip log have second word lowercased, except for "Health Status". This changes it to make it inline with all other fields again.

This allows log parsers to always extract correct information about the "artist" and "title" fields: previously that was a problem if either (or both) of those fields contained a "-" character.

* Include MusicBrainz Release URL in log output
This also passes *all* metadata to the `result` object, giving loggers a
lot more (release) metadata to work with, in case custom, “3rd party”
loggers (or even ourselves in the future!) want to do something more
fancy or expansive with the metadata in the log file.

* Uppercase "url" in output: "URL"

In general, "albums" in everyday usage is what on MusicBrainz would be
considered a release group[1] while as far as I can tell, every single
instance of it being used in whipper is referring to a single "edition"
of an album, which is what would be called a "release" in
MusicBrainz terminology[2].

The values stored in variables _inARDatabase and _accuratelyRipped were double incremented when both AR v1 and AR V2 entries were present e/o matched, leading to a wrong calculation and potentially erroneous summary reporting.
After this fix, in case of double matches only one is counted against each track.
This commit also includes:
- whitespace / code formatting fixes
- slight syntax related changes: except <exception_name>, e -> except <exception_name> as e
- 3 pointless instructions instances have been rewritten [sorted] (spotted by semi-automatic check)
The unrelated changes shouldn't have any real impact on whipper's behaviour.
- output path no longer has fallbacks
- refactor accuraterip cache
- use requests to download accuraterip entries
- add tests for accuraterip functionality
- remove gobject support from accuraterip-checksum calculation
- default track template now includes extension
- begin to remove support for continuing rip
- begin to use print instead of sys.stdout.write() throughout
